I'm Ready Take Me To The Commitment Form

Please read the following instructions thoroughly before submitting a commitment form.
First only a DESIGNATED OFFICIAL such as an Organization Athletic Director should be submitting ALL of his/her clubs commitment forms. For Accuracy ATHLETIC DIRECTORS SHOULD BE FILLING OUT THIS FORM! They MUST be each submitted individually meaning one per team just like the paper forms. Most FIELDS ARE REQUIRED TO HAVE INFORMATION BEFORE YOU MAY SUBMIT. In the Blocked date/Comment box a club is only allowed up to One (1) blocked date. You do not have to have information in this field. If more are required you must submit and speak to the NEPL SPORTS DIRECTOR, BILL TEES.

 PLEASE NOTE
This field may also be used for additional comments such as if you would like to receive a call or if your starting time is not on the drop down menu. For example a start time of 9:45 AM is not listed select 9:30 and add in the Blocked dates/comments field: Actual start Time is 9:45 AM. This should also be used also if you wish to submit an alternate field/court day & time.

FIELD/COURT LOCATION please be as specific as possible for example: Solly & Enfield, not just Meadows Field. Make sure your spelling is correct.

IMPORTANT
BLOCKED DATES. These should be dates your teams will not have enough players to compete due to school or religious functions not because a team won't have certain players that day. This includes Graduation (players, not family & friend's), Communions, school trips, etc. Please remember, these exceptions will only be allowed if the number of players affected would bring your total number of available players below the number needed to field a team. Obvious dates being blocked by the NEPL will include the Memorial Day Weekend. This means you do not have to submit these as blocked . Again, you can also put comments in this box. Please note the NEPL will Honor Blocked dates submitted Prior to Schedules being made So have your coaches check with the school & players.
VERY IMPORTANT
When you are sure everything is correct click the submit button. You WILL be required to type in verification characters which appear on your screen. Upon each submission you will receive a thank you and be directed to either submit another form or to exit the site. When you receive the Thank You you submission has gone through.

EXTREMELY IMPORTANT:
 The NEPL requires that the PRESIDENT, ATHLETIC DIRECTOR & CLUB
BASKETBALL COMMISSIONER Fields be filled in or the Form WILL BE REJECTED!
